Who we are. EliteUSMD, Inc. (“EliteUSMD,” “we,” “us,” “our,” or the “Company”) is a physician-owned professional medical entity. EliteUSMD holds all physician and provider relationships and is responsible for clinical oversight and for the Good Faith Examinations performed through our network.
Health Covers Inc. is a separate management services organization that provides administrative, credentialing, technology, and billing services to EliteUSMD. Health Covers Inc. does not practice medicine, does not provide medical advice, and does not control clinical decision-making. All clinical decisions are made by licensed providers exercising independent professional judgment.
These Purchase Terms govern fees, billing, and cancellation for the EliteUSMD program. They form part of, and are incorporated into, the Terms of Use.

There is no placement fee. Fees are stated in U.S. dollars and are exclusive of any applicable taxes.
These figures must match the current signed clinic agreement exactly. If the agreement and this page disagree, the agreement controls, but a public page showing different numbers is a problem. Confirm before this goes live.
Billing and payment
Onboarding and the first month of the program fee are due at signing. Monthly program fees bill in advance on the same day each month. Good Faith Examination fees bill in arrears based on examinations actually performed.
By providing a payment method or completing an ACH authorization, you authorize us to charge all fees when due. You are responsible for keeping payment information current.
Payments are processed by a third-party payment processor. We do not store full payment card numbers. Card payments are handled in accordance with PCI-DSS.
Late payment
Invoices are due on receipt unless stated otherwise. If a payment fails or an invoice remains unpaid, we will notify you and attempt to resolve it.
We may suspend access to the Services for non-payment after written notice and a cure period. Suspension does not relieve you of fees already incurred.
Suspension will not be applied in a way that leaves a patient mid-examination without a resolution.
Term and cancellation
The program is month to month. There is no long-term contract and no termination fee.
Either party may cancel with written notice. Cancellation takes effect at the end of the then-current billing month. Fees already paid for the current month are not refunded, and examination fees already incurred remain payable.
On cancellation, you remain entitled to obtain the medical records generated for your patients as required by law.
Refunds
Program fees and onboarding fees are non-refundable except where required by law or where we fail to provide the Services.
If you believe you were billed in error, contact us within 30 days of the invoice date and we will investigate. Verified billing errors are corrected or credited.

Taxes
Nothing in this policy limits your right to contact a state medical board, a licensing authority, a consumer protection agency, or any other regulator at any time, whether or not you have raised the matter with us first.
