Who we are. EliteUSMD, Inc. (“EliteUSMD,” “we,” “us,” “our,” or the “Company”) is a physician-owned professional medical entity. EliteUSMD holds all physician and provider relationships and is responsible for clinical oversight and for the Good Faith Examinations performed through our network.

Health Covers Inc. is a separate management services organization that provides administrative, credentialing, technology, and billing services to EliteUSMD. Health Covers Inc. does not practice medicine, does not provide medical advice, and does not control clinical decision-making. All clinical decisions are made by licensed providers exercising independent professional judgment.

Our position

EliteUSMD, Inc. does not sell personal information, and does not share personal information for cross-context behavioral advertising.

We have not done so in the preceding twelve months, and we have no plans to. There is no revenue model here that depends on your data.

We do not disclose personal information to advertisers, data brokers, or marketing partners. Protected Health Information, including Good Faith Examination records and video recordings, is never sold, licensed, or disclosed for any commercial or advertising purpose.

This page exists because California law requires us to provide a clear mechanism for you to make the request, whether or not we sell anything. You are welcome to submit one.

Your rights

If you are a California resident, you have the right to:

Residents of other states with comparable privacy laws have similar rights. We honor them regardless of where you live.

How to submit a request

Email info@eliteusmd.com with the subject line “Privacy Request.”

Include your name, email address, phone number, and the nature of your request. We will verify your identity before acting, and will respond within the time required by law, generally 45 days.

An authorized agent may submit a request on your behalf with proof of authorization.

Medical records are handled separately

Requests relating to protected health information — including access to a Good Faith Examination record or recording — are governed by HIPAA and applicable state law, not by CCPA.

Patients seeking a medical record should contact the treating clinic, or contact us at info@eliteusmd.com and we will direct the request appropriately.

We cannot delete a medical record where retention is required by law, and we will tell you plainly when that is the case rather than quietly declining.
